Addr./Phone Private Yes in this field indicates that the employee has requested that their home phone and address be kept private. No indicates that the employee does not mind if the home phone and address are published or released.
Home Address & Home Phone These fields display the address and phone number the employee has noted as their mailing address/residence. If Yes displays in the preceding field, the information in this field will display in italics for an Employee within the viewer's department to remind the viewer not to release the Employee's address and phone number. For an Employee outside the viewer's department, Yes in the preceding field results in "private" displaying in these fields.
I-9 Date This field displays the date on which the employee's I-9 was completed. The I-9 is the federal form verifying eligibility to work. For foreign nationals, you may see something like E20010315, which means their visa or other permit expires on that date (in this case, 03-15-2001).
Birthdate The date on which the employee was born is displayed in this field.
Highest Ed. Lvl This field indicates the highest degree earned by the Employee, if this information is known.
Visa Permit If applicable, the type of visa and the dates associated with it are displayed in this field.
Visa Status If applicable, the visa status displays in this field.
